If my family member is coming from a hospital, what should the hospital or I provide for his or her admittance to MediLodge of Howell?

For admittance to MediLodge of Howell directly from the hospital after a 3-day qualifying stay, the hospital discharge team will communicate directly with our Admissions team, making the process much easier for you and your loved one. We encourage you to come tour our facility. Once you decide to come to MediLodge, we ask that you bring the following...

• a Medicare Card and at least one other insurance card
• a Picture Identification
• the following papers, as applicable: DPOA, Patient Advocate, Guardianship, Conservatorship
• 5 to 7 days worth of clothing
